Any fees for services provided not covered by the insurance company will be your responsibility. You are responsible for paying applicable co-pays, co-insurance amounts, or deductibles at the time of service.
Self-Pay Rates for Psychological Testing
At Lee Therapy and Assessment, PLLC, we realize that seeking comprehensive psychological services represents a significant investment of not only time, but also money. All costs will be explained upfront, allowing you to make informed decisions concerning your care. While our fees are outlined below, don’t hesitate to contact us to discuss options for covering the cost of services. Together we will come up with a solution that is best for you.
The standard fee for comprehensive psychological testing at our practice is $1,500. This flat rate covers all aspects of the evaluation process, including initial interviews, test administration, scoring, interpretation, and a comprehensive written report. This fee applies to all types of evaluations, including those for autism spectrum disorder, ADHD, learning disabilities, and other psychological or educational concerns. Costs do not change despite the client’s age.
Sliding Scale Options:
We are committed to making psychological testing accessible to all clients. If you are unable to pay the full $1,500 rate due to financial hardship, we offer a sliding scale fee based on your household income. The sliding scale ranges from $750 to $1,500. To determine your eligibility and assigned rate, you will be asked to provide your yearly income amount; however, you will not be required to provide tax information or pay stubs. If you wish to be considered for the sliding scale, please let us know. We will review your information confidentially and inform you of your assigned fee before testing begins.
Payments and Policies:
Although we do not offer payment plans at this time, we do understand that testing can be a large up front cost. We offer split payments, meaning that you can pay in 3 phases – 1/3 at the intake appointment, 1/3 at the face-to-face testing session, and 1/3 at the feedback session. This applies to sliding scale fee arrangements as well.
For clients who request it, our office will provide a superbill to clients. This superbill can be submitted by a client to their insurer for out-of-network benefits. Many insurance plans offer out-of-network benefits, and the client may be directly reimbursed some portion. If you choose to discontinue testing for any reason, you are still required to pay the full fee for testing.
Types of Testing / Assessments We Offer
Autism Assessment - We provide comprehensive evaluations to determine whether an individual meets criteria for Autism Spectrum Disorder (ASD). Our assessments include standardized testing, observations, and interviews to understand social skills, communication, and behavior patterns.
ADHD Testing - Our ADHD assessments use a combination of interviews, behavior rating scales, and cognitive tests to identify symptoms of Attention-Deficit/Hyperactivity Disorder and help guide treatment or support strategies.
Personality Assessment - We use evidence-based questionnaires and interviews to assess personality traits, emotional well-being, and coping styles. These insights can help inform mental health treatment and personal growth.
Achievement Assessment - We offer academic achievement testing to measure skills in reading, writing, and math. These assessments help identify learning strengths and challenges, supporting educational planning and intervention.
Giftedness Testing / Twice Exceptional - Our evaluations identify gifted students and those who are both gifted and have learning differences (twice exceptional). We assess intellectual abilities and academic skills to support individualized learning needs.
Behavioral Evaluation - Our behavioral and emotional assessments evaluate concerns such as anxiety, depression, mood, and behavior. We use interviews and rating scales to help understand and address emotional or behavioral challenges.
Developmental Assessment - Our developmental assessments examine milestones in areas such as language, motor skills, and social interaction. Early identification of developmental delays can help guide effective intervention.
Vocational / Career Assessment - We offer vocational and career assessments to help individuals explore their interests, strengths, and career options. These tools can guide educational and occupational decisions.
We do not offer competency to stand trial evaluations, fitness to proceed evaluations, insanity evaluations, fitness for duty evaluations for law enforcement personnel, or child custody or adoption evaluations. We may not sign the Declaration of Psychological and Emotional Health form required for any individual seeking a personal protection endorsement under 37 TAC §35.91.

Cancellation Policy
Out of courtesy to our clinicians and other clients who are waiting for an appointment, please call as soon as possible to cancel an appointment. At Lee Therapy and Assessment, PLLC, we understand the nature of emergencies and will work with you as long as things are communicated clearly and in a timely manner. However, appointments may book far in advance and, at times, are difficult to book with short notice.
Subsequent appointments made after the consultation visit must be cancelled with 48 hours’ notice. If you are unable to reach the office, leaving voicemails, e-mails, or texts timestamped before the 48-hour time frame are not charged. Due to the nature of psychological testing, 4-hour windows are generally set aside for the evaluation period. Due to the longer time frames set aside for these appointments, no-shows and late cancellations will incur the following fees:
Cancellation in less than 48 hours of appointment – $200 fee
No-show for appointment – $250 fee
All fees listed above must be paid prior to any re-scheduling of the missed appointment.
Good Faith Estimate
Beginning 01/01/2022 under sec 2799B-6 of the Public Health Service Act, behavioral healthcare providers and medical facilities are required to provide individuals whom aren’t enrolled in an insurance plan or a Federal healthcare program with a “Good Faith Estimate” (GFE) explaining how much your care will cost.
You have the right to receive a GFE for the total expected cost of any non-emergency items or services. This includes costs like medical tests, prescription drugs, equipment and hospital fees.
Make sure your provider gives you a GFE in writing at least 1 business day before your service. You can ask your provider, and any other provider you choose, for a GFE before you schedule a service.
If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
